What insurance providers do you accept?

We participate with many insurance plans including BCBS, Aetna, Medicare, Medicaid, HAP, Humana, United Health Care, and more. Call (586) 228-2882 to verify your specific plan.

How do I cancel or reschedule my appointment?

Provide a minimum of 24 business hours notice. Failure to provide notice results in an Open Time Slot Fee: $75 (new patient), $50 (EMG), $30-$60 (follow-up).

What should I bring to my first visit?

Current health insurance cards, photo ID, credit card for co-pays, completed Medical History Form, all recent test results, shorts/tank top for exam, current medications, and records from your referring physician.

How do I request a prescription refill?

Submit a refill request through our online form. Please allow 48-72 hours for processing. The practice only refills medications they prescribed.
