What to Bring

• check_circle All recent test results (MRI, CT, X-ray, blood work)
• check_circle Shorts and tank top for physical exam
• check_circle Current medications or medication list
• check_circle Records from your referring physician
• check_circle For minor patients: parent or legal guardian must bring own driver's license. Proof of guardianship required if not parent.
