Appointment & Prescription Policies

Referrals

If your insurance plan requires a referral, your primary care physician (PCP) will provide the referral. Referrals are valid for a limited number of visits and a limited time period. It is YOUR responsibility to keep track of how many visits remain on the referral and when it expires.

Prescription Refill Policy

Our practice will ONLY refill medications that we have prescribed. Please bring all medication bottles to each visit. Controlled substances will NOT be refilled after hours or on weekends. Please request refills at least 3 business days before running out.

No-Show / Late Cancellation Policy

Please provide a minimum of 24 business hours notice if you need to cancel or reschedule your appointment. Failure to provide adequate notice will result in an Open Time Slot Fee:

• New Patient Appointment: $75
• EMG Appointment: $50
• Follow-up (first occurrence): $30
• Follow-up (subsequent): $60

Refusing to pay applicable fees may result in termination of the physician-patient relationship and may prevent:

• Phone medication refill requests
• Doctor phone consultations
• Scheduling follow-up appointments
